Not being able to get up from the floor is a marker of frailty but also a real problem. lots of ambulances get called to help someone who has fallen. i heard last week of a poor chap who fell one evening putting rubbish into his bin - he lay on the cold floor all night - neighbours spotted him in morning and called an ambulance that took another 2 hours to arrive. they wrapped him up - kept him company but were afraid to lift/move him.. hes still in hospital 10 days later. in his house he might have had a button to press for help and ive certainly heard of iPhones or similar with drop detection calling an ambulance if not cancelled but the actual lifting of someone is hard. Im met many an elderly wife who couldnt lift her husband from the floor. Is there a simple gadget that could help? some form of inflatable device or simple to rig up pulley system?? even just getign someone upright and off the cold floor might be enough… almost a life jack.. ive heard of expanding horse riding jackets for falls? just a thought

looking for advice on webcams!

looking for advice on webcams! we have a meeting room with a plasma screen on the wall. using a laptops HDMI we can get a teams call on the big screen and sound from the speaker on the TV so everyone can hear and see without having to crowd around the laptop - but the laptop camera is on a table and doesn’t pick everyone up - we are looking for a webcam that we can put on top of the TV - with a wide angle so it show everyone in the room - (we are all sitting spaced apart!) presumably with a long usb cable so I can plug it into the laptop. if it had some sort of clever mic would be good however ive found a usb omni directional mic that sits on the table in the middle of the room.. anyone got any kit suggestions? needs to work with Teams!! #webcams

OK for my NHS colleagues who are on the non-cloud version of outlook: are you struggling to create a Teams meeting in a channel? there is no calendar app in Teams showing up and there won’t be until your account has been upgraded to the cloud.. however there is a workaround. Go to your channel - click on meet now - then select the … and choose the meeting properties menu - in there you can copy a Join Meeting tag to the clipboard. you can then create a meeting invite in calendar as normal and paste this to the bottom of the invite - People will then be invited to the correct channel rather than it creating a new meeting each time. This is useful when you are saving the minutes and agendas in the channel for easy access. #nhsteams

Apple might sell lots of iWatch6s because of Covid. If you get it & you get ill, one of the issues is something called silent hypoxia. You can get very low in oxygen and not really know it. By the time you realise you might be ill enough to need ventilation. thats bad as only 50% survive ventilation. The trick therefore is to catch people who are deteriorating early & treat them with Oxygen, Steroids and perhaps CPAP all of which have much higher success rates (because the person isnt so ill). To this end we are hearing that NHSE have bought hundreds of thousands of pulse oxs. The idea is that anyone who gets covid gets lent one & monitors their SaO2 daily and if it drops below 93% they go into hospital for treatment. In a trial in Manchester the mortality was reduced from 28% to 1% just by detecting deterioration early & treating hard. Locally we are rapidly looking at how we implement this. We will probably be using a Florey in accuRx to help us, we could do with some help improving it Jacob Haddad) and it looks like we can monitor what’s going on across the region (& possibly the UK) using Edenbridge Healthcare Limited’s Apex product. Will there be enough PulseOxs for everyone? What a clever time to launch a watch with one built in! PS I’m open to testing one :-)

The Flu Manager report in Apex - allows you to see how your practice is doing or if you enable data sharing you can track any practice in your PCN or federation or CCG or indeed look how a locality is doing. Report tells you how many you have done how many others how many have opted out and there is a breakdown of which LTCs people have. It’s laudable aim is to try and collect as much info for LTCs in the waiting room as possible they also have a HomePOD for nursing homes etc. The backend questionaire building software looked really good and flexible tempted to give it a go. #primarycare #gp #pcn #digitalhealth

Sensitivity and Specificity

From my LinkedIn Post

For the less scientific it might be worth revisiting the concepts of sensitivity and specificity.

The ideal test should be 100% accurate, it should be specific ie never say you have a disease when you don’t (low false positives) and sensitive ie never say you don’t have a disease when you do (low false negatives)

Now very few medical tests meet this 100% goal. The classic example that people might be familiar with is the urine pregnancy test – if it says you are pregnant you almost certainly are pregnant, but if it says you aren’t, there is a chance you are so check again..

Now Covid testing is interesting as although the experts say the lab bit of the test is almost 100% there is a huge human factor involved in the test especially in self testing or through a car window. If you don’t insert the swab far enough in or get enough of a sample the false negative rate can apparently be high.

What does this mean & what is the implication? Well it means if it says you have covid – you almost certainly do.. but if it says you don’t, well you might still have it.. especially if you have classic symptoms – temp and cough..

So my advice is don’t rely on a negative test to assume you are safe to go back to work or see vulnerable relatives, which is what some of my patients are doing.

For the last few years on and off Ive run an informal course/workshop for local GP registrars on “the business of general practice” going through things like the structure of NHS, commissioning, the GMS contract, the partnership model, how GP accounts work, working in groups, different types of practices, how to choose a practice to join, how to change a practice, what to put on CV/say at interview, portfolio careers. etc… Some are asking me to do it this year and of course I might have to do it on line..“ “ “My question - are any of my contacts on here interested in joining/listening in or having bespoke sessions for them? (I have previously done in person whole day sessions for Pharma colleagues) Is this something that only appeals to GP registrars or do existing GPs? PMs? perhaps other clinicians or perhaps staff from companies working with NHS might be keen on)” “ “If interested would you be prepared to pay a small amount so I can get proper conference software and dedicate the proper time to it? “ “ “If you are interested what bits are you especially interested in. Very excited - Edenbridge Healthcare Limited have just launched their new Clinician Appraisal report as part of their Apex product. I think its amazing (I helped them with it!!) Im on hols this week but I will do a screencast explaining it when im back. ITs a tool that easily allows a clinician to compare their activity with themselves over time (useful for registrars) with others or groups. Great for appraisals… more to come

Now Docman has gone down - for 5 practices locally that I know of - not sure if its a national issue or a local network one - this just happens too often - this week - this week we have been without major IT systems for large periods of time. Would you fly with an airline that had such problems?? Is it a local underinvestment? is it poor hardware / software ? never really get an answer? is it just too many links in a chain.. Its .. a router broke ,, or an upgrade didn’t work, or a workman cut a line….. #despairing

The end of online private GPs?

Does the rapid adoption of video consultation by primary care spell the end for private IT companies trying to take over NHS practices? Now all the GPs at my practice are happy to do video consults why does a patient need to go elsewhere? Part of the reason was old type surgeries would make people come down and if you led a busy life you didn’t want that - well you can get that with me now. Part of the reason was you get seen quicker well with total triage in place I will see you in minutes“ “ “What we have is an interesting juxtaposition - Commissioners are moving towards place based care - primary care homes - at population levels of £30-50K but we have just demonstrated that I can now access any patients record anywhere and I can consult them online. I could “sell” my care nationwide. Plenty of patients who used to live in my area begged to stay on my books, but I couldn’t given the way general practice has historically worked with practice boundaries.“ “ “Is it time to get rid of the boundary? get rid of the entitlement to a home visit the major stumbling block to taking on patients outside your area? “ “ “Are we going to get NHS GP practices that look after mobile IT literate patients (anyone want to join me in an online Superpractice?) and practices based in an area? #primarycare

Been looking at Urgo Medicals new wound care app thanks to Daniel Barnes. They are using software that I haven’t seen before that watches users using versions of the App and creates heat maps etc of their interaction/touches with the app having been given tasks to do.. Their laudable aim is to create the easiest to use app as possible.

Thinking about DriverNet again - while I think it would work well as an app used in GPland to place GPs on calls/homevisits. Could you use it the other way around? At moment lots of practices are asking pts to visit hot hubs and asking them to beep when arrived - some have long queues forming some not“ “ “could you add the patient as a driver to the app - and track them to the base and be notified when they arrive? could you use this as a form of queuing system? Should they be using this nationally for the testing to smooth the flow/reduce waits??

Followers of mine will know I’ve long been after a tool that would allow me to take a picture on my smartphone of the patient in front of me and have it appear in the medical record with as little fuss as possible. PhotoSAF from Consultant connect comes close..but uses a website that someone then has to match to the patient and attach. Others tell me they have solutions but none ever quite do what I’d like - recently I’ve been using accuRx to send pts links so they could send a photo of themselves - It strikes me with only a little change this could be the feature Im after? - Have a photo button on the accuRx toolbar - send a sms to my phone with the link - let me take 1-5 photos and have them upload them into the current patients record Jacob Haddad - thoughts???“ “ “#primarycare #gp #generalpractice

Nominating Chemists

Changing nomination temporarily in EMIS is a right pain! Locally pushing electronic nomination hard to reduce numbers coming in. However which chemist a patient is happy using for regular repeats and who they might want to use for an acute prescription is different.. however its a right pain to change then change back!

Trying a Standing Desk

Ergotron sent me a standing desk to try. Full disclosure the desk was free but they neither requested a review or insisted on a positive review.

I’ve always wondered what my patients would think of it, my wife also a GP who has one says only a few ask & her bad back works as a good reason for hers

At the moment I’m working from home doing telephone/video surgeries every day and sometimes it just feels like I’m sat 12 hours a day – so I immediately said yes to the offer – what was the worst that could happen – some wood for the bonfire?

Well it takes a little getting used to. I’ve a dual monitor setup so they sent me a big one and its big. It came on a pallet. But it kind of works on my antique desk. In sitting position the monitors are a little higher than usual but that’s not a problem and in standing position it actually works quite well. I find that I use it for half an hour a surgery just to get me up out of the chair. This may sound really sad but im not used to standing for hours any longer gone are the days of 5 hour ward rounds.

Where it really works is on Team conferences where I can actually move around with my video off. Whether I’ll use it in front of live patients I’m not sure but as the future holds more phone consulting I can see me using it going forward!

How 2 Launched

In response to the Covid19 crisis we have launched our Howbeck How2 online video resource to all GP Practices for free for the next 6 months in an effort to support our clinicians and practices during the current pandemic. Our current content (more coming) relating to understanding EMIS Web and Docman10 is particularly useful with many staff now working from home and to support recently retired and former front line staff returning to practice to enable them to get up to speed on primary care systems. As practices are looking to work together more, our bundles on remote consultations will help support you in this.

This site was being developed for release in the Autumn to support Practices & PCNs in optimising the use of IT systems in General Practice, particularly wrt Additional Roles Requirement of the PCN DES. The content has been produced by our experienced team of clinicians & trainers in primary care systems, but isn’t complete yet – more is coming. If you have a video on a topic that we haven’t covered that you are willing to share on this site during this time, or want to help create content please get in touch.

Please share and let all your colleagues know that this resource is available to them.

Should gps work in soundproof cubicles (or from home) and only ask pts to come down to special exam rooms if absolutely needed? (a little like the self cleaning toilet that showers itself) do we need a radical rethink of GP property? “ “ “When I was training as a GP - the RCGP view of the world was to sit at corners of a desk to each other - not across the desk that’s too dictatorial and paternal. FTSU

Our Freedom to Speak Up (FTSU) Guardian was scheduled to present at the National Guardian’s Office Regional Conference today. Due to the Covid crisis, it took place via webinar, as it was felt important that GP Practice staff know they are fully supported when it comes to reporting concerns about patient care and the safety of patients and colleagues.

Following the introduction of the Guardian roles in Primary Care in 2017, it struck us that General Practice is unique in how the role can be provided. As small organisations it is not practical for practices to have an individual person, independent of management to represent staff. What is required is a cost effective service deployed with an understanding of the uniqueness and sensitivities of Primary Care.

Howbeck is now delivering a FTSU service to over 100 of our clients inc GP Practices, PCNs and Feds. We support them to embed the appropriate policies and promote the service to make it visible to staff, which is critical for success.

A need for a patient side results aggregator?

Do we need a common conduit/protocol/messaging system for home results? Let me explain - with the rapid adoption of video consultation and home based patients how do I receive readings from a patients medical sensors? Lets say they have an iWatch or similar - how do they send me their heart rate? their ECG? their pulse Ox? they could have digital scales, BP machine, Glucose monitor? home blood testing kits read through a smart phone, home urine tests read through a smart phone, thermometers etc. “ “ “It’s going to be pain to make users read out or email the results every time? how do I “ping” them for the results and with their permission get them in my clinical system or in an app linked to it? “ “ “how do I do this while on a video call? how do I do it if remotely monitoring them? lets say I just want to check how Ethel’s vital signs were this am? “ “ “Is there a smartphone app that sits on a patients phone and acts as a results aggregator and dispenses the results to authorised health care professionals in coded form? GP@home

Over the last 24 hours I’ve been having a go with Paul Bensley’s Surgery Connect and their GP@home. It solves the “I don’t want to use my own phone to make hundreds of calls to patients” problem. The system which a practice can use for inbound calls, allows users to make outbound calls through it. At the moment the system rings the users mobile then the patients and connects the two. The patient sees the CLI of the surgery or whatever you choose. The cost of the call is picked up by the system not the user at home so is billed to the practice. I’ve only scrapped the surface but it logs all calls and records them too so you can attach them to the patient record if you choose. My understanding is this week they are launching a soft phone version which allows clinicians to use a headset and their computer rather than their own phone to make the calls. Of course this might be bandwidth dependent.“ “ “It also appears to allow home workers to operate in hunt groups etc and answer the phone as if sat in the surgery which might help as staff self isolate.” “ “Really impressed. It obviously needs some setting up but Paul’s team are ready to do this. Post Covid infection control changes

Just been in a meeting talking about infection control changes that might need to persist post covid.. we think it could be the end of the touchscreen check-in - they must be a real contamination hazard - why hasn’t anyone created an app that allows a patient to check-in when they arrive at the surgery? It could be geo-fenced to make sure people don’t check in when they aren’t in the practice. Use GP@home from X-On to record trainees doing telephone calls

It strikes me the the recording you call functionality in GP@home from X-on could be really useful for training GP Registrars and others inc ANPs etc. We currently do sit in surgeries but the tutee is always on their best behaviour - if you made them use it for a few surgeries you could pick consultations at random to listen to. this might be a use for a surgery even if they didn’t use X-on’s other services?“ “ “#gp #practicemanagement #primarycare #GPtraining” “

Had a demo of DriverNet from Anthony Chisnall yesterday and I have to say I think it could very easily turn into a useful tool for larger general practices and PCNs. It very simply allows a receptionist or coordinator to assign visits to whoever is visiting and send all the info needed about that visit including a PDF summary of the notes. It has a load of useful features and perhaps quite a few not needed at practice level. (but I believe these are easily turned off) Very small practices might not see the need, but if you are running a larger one or trying to setup a whole PCN service with part time staff including home visiting ANPs, paramedics and perhaps covering multiple sites I think you would find it useful. It needs some UI tweaks perhaps but it could be deployed in days if the IG checks out. Its only real competitor appears to be Doc Abode which has a following. ok here is an idea for an app!

ok here is an idea for an app! all the thinking is very soon there are going to be lots of home visits happening - of ill people needing a visit or post hospital patients needing followup. while I’ve heard of some systems for monitoring people at home - I think there is a gap in the market for a scheduling app. A receptionist at one or more practice logs into a website - has a group of clinicians GPs ANPs etc who are visiting that day - they add a patients name address phone and problem/urgency. perhaps they allocate - or allow people to select/choose. perhaps some AI re who is closest seen before etc. the GPs out on visits - get the info on who to see. link to sat nav - details to ring/ what’s wrong. sent a PDF of their notes to use. All needs to be secure/IG compliant - nothing kept on phone/iPad. some lone worker stuff around tracking/safety send help? messaging back to base… perhaps record a consultation in free text and have it sent back to surgery for upload to record. This type of thing must exist for large companies with field workers. The clever idea is to have patches. some times of day practice based with practice GPs. if practices agree to work together then link allow multiple receptionists to post visits and pool to reply - could have working at whole CCG level?

We are thinking about how we might examine someone through glass or over a CCTV/video link. We invite them into a room but our clinical team stays next door and only goes in for the bare minimum time. Is there any kit that would facilitate this? ideally we would like the patient to self use a pulse ox, bp, ?stethoscope and us in the room next door see the results/ hear the auscultation. if we had an iPad or similar would bluetooth kit work through a wall? window? any thoughts?

GP connect is huge - it appears to let 3 things happen. 1. 111 can see a full medical record on anyone. 2. they can book patients direct into a practice 3 they can send discharge and or contact info. now these all sound good. but we have Summary care record, we have document interchange for discharge and some areas were sorting booking. The interesting thing is they have forced it through - practices dont have to turn it on, no one can object.. They also havent given any info on who can use it and will they turn it off? will they find reasons to let others have access,, social services, police, MI5, DVLA, Customs and Excise, DWP.. There is a real danger here that anything you tell your GP will get out. Im sure no one minds a consultant in ITU accessing their records,, the a random member of staff who lives across the road.. what audit trail? can I opt out? can I see what they see?“ “#gp #primarycare #digitalhealth #nhs

good idea I’ve heard today - dial a runner. For patients obsessed by running or cycling where an hour a day isn’t enough - volunteer to be a drug mule and collect patients who are self-isolating or shielding from the pharmacy and run/bike it to them - using some form of precautions - presumable post it through letter box. Can UBER/ just eat/ deliveroo adopt their software to allow people to volunteer and be available to run..and allow people to put out requests?

Im spending the morning doing a load of telephone assessments on people. While I can gather some information by asking questions and listening to how breathless people are and I can sometimes gain more by switching to a video call and looking at them it would be really helpful if people could do some basic observations at home. without wanting to cause a round of panic buying - here is my list of stuff it might be worth having as a minimum at home for remote assessment, thermometer, pulse oximeter, blood pressure machine that measures pulse and detects AF, smart watch or other way of doing an ECG. Can anyone think of any other essential kit?

This came in yesterday - I almost miseed it“ “ “GPs can certify death via telephone under new emergency legislation - 26 March 2020 Notes by By Helen Quinn” “ “The Coronavirus Act, which was passed by Parliament yesterday and is law as of today, means it is now possible for GPs to certify death by telephone.” “ “The emergency laws are designed to consider the coronavirus restrictions on in-person interactions and that death may occur in households that are self-isolating, making it difficult for GPs to attend in person.” “ “The new Act, which extends to over 300 pages and is valid for two years, covers law changes for the healthcare system, including emergency registration of health professionals; health service indemnification; NHS and local authority care and support; and includes new information about the registration of deaths.” “ “It modifies, temporarily, the Notification of Death Regulations 2019 regulations, which came into effect in October last year.” “ “From today, GPs have up to 28 days to attend a death instead of the usual 14 days. OK some areas/PCNs/practices are setting up Hot centres and Cold centres and telephone triage centres. Ie one building acts as one - one as another and potential a third doing the triage. This is partly to keep patients apart but also to protect staff.“ “ “In my area we are EMIS and cross org appointment booking which works well - one site can book any pt from any practice into any of the sites - The problem is there is no easy way of staff working at multiple sites other than adding them individually - we need some sort of super surgery or cross practice logons - I’ve heard of some sites using dummy logons for multiple users….” “ “The real pain is you cant send prescriptions via EPS using enterprise consultations! Perhaps its too early to be thinking these thoughts but general practice has just had a huge shake up and will be changed for ever. Ok - there is going to be a lot of catchup work to do once this is over - the amount of routine monitoring we aren’t doing at the moment is amazing - makes you think how much of it was worthwhile (but those that have read feynmans account of the shuttle disaster might recognise that as faulty thinking) We have seen massive adoption of new tech that I think will change general practice - how much can be done over the phone or on line - loads! How many requests for consultations aren’t really needed and while we have an excuse to say no we can.. can we hold our nerve and keep saying no… how much home working could be done? how much do you need a GP in an area? this is perhaps fundamental. most NHS services are place based but we have just shown with access to records, EPS for prescriptions I can be on a tropical island and consult effectively. Do I need to be local to the patient? or vice versa can my surgery expand its offering and offer services outwith of its boundaries?

Relationships are key!

It’s odd that our GP clinical systems hold so little information about relationships. When we get safeguarding forms to fill it - its sometimes the only time that you understand who is related to who and how e.g. adopted, fostered, living with, step child etc. Also other relations - recently a well known patient of mine died. She must be friends/related to half the town and while I know her son and daughter Im a lot less clear on who is a cousin, in law, friend neighbour. I guess in terms of GDPR you might say this info is irrelevant to medical care, but with contact tracing and isolating in a home it becomes more relevant, who is living with who? do any of the other clinical systems do a better job of this?“ “ “#gp #primarycare #practicemanagement

However some tell me its a struggle to get the people who might need their help to hear about their offer. Some like me post on here but not everyone is on here. Would it be worth NHS digital or similar collating an online list of offers? So that CCGs/CSUs/practices can see who is offering what? Where are others posting? Met with Chris Pruvey who has developed a really interesting online physio/msk home exercise platform. As a GP or ANP - you see someone with a MSK issue. You want them to have physio - but you know waits are huge so you give them an online customised course of exercises to do based on their diagnosis. Its similar to sheets that physios often give out but it demos the exercises and explains them and checks compliance by counting logins. I’ve seen similar from others and in other fields but this is neat and simple. Chris reckons it reduces reattandance and improves return to work.. Todays issue: Need to get soft phones working over the VPN/VDI… Lot of people sat at home trying to consult with patients. OK some will be video calls.. but a lot are telephone. Docs and nurses don’t want to use their own phones to use the calls either from a cost pov or a giving our their CLI. We have been doing a project with Dan Worman from Refero where we have put a 'webchat'…

We have been doing a project with Dan Worman from Refero where we have put a “webchat” box on our website. We have had it running for a couple of months. We havent particularly advertised it but use has been picking up. It allows patients to contact us without emailing or ringing. Its not 100% live yet as we ask for up to an hour to reply (so i wont call it a livechat). We have quite a bit of learning as to what people use it for - in the last 48 hours its gone mad - with people asking questions. The good news is we can boiler plate replies and the system allows us to hand off messages to other members of the team - so meds queries can go to the pharmacy team - our ANPs are on it now. ok - remote working is the next nightmare. locally we use horizon from vwmare. Most days it (our local server) can cope with about 10 users, it works well when it works. Ive for years been saying the NHS should move completey to a VDI infrastructure…. “ “ “So how do you get hundreds of people securely working from home when they have to log on to n3? Does EPS need a facility for an urgent script?“ “I love EPS, reduces signing, reduces paperwork, makes the pathway electronic. Im on a medicine, I use it - I use an app - request my med, it gets sent to a online pharmacy and it arrives by post in a couple of days. What do you do if you are acutely unwell? On many home visits - ive turned up to see a house bound person who is in need of meds, i hand over a prescription and they are house bound… hopefully a friend/neighbour/relative can go get it. Ok I now go back to the surgery and send it electonically to their nominated chemist (would be even better if i could do this from an app at bedside) but increasingly big chemists arent delivering same day. some will… not all. and its variable. here i am trying to keep people out of hospital but cant because i can get them any meds! To go back to practice - ring round 5 chemsits - findd out who will deliver today - change the nomination to them - send it - then ?change the nomination back as was there a reason why it wasnt them….. or do we need a way of temporaily sending it to a urgent deliverer ~(perhaps they get paid more - perhaps this is a service some might offer?) OK - we need to expand EPS to include sicknotes…or similar“ “ “An explanation. My clinical system lets me print out sick notes. But I have to manually sign them. This is a pain when we are triaging all calls and dont want people to come down to the sugery. I could post it.. but why should I pick up the cost. We arent allowed fax machines… and to print it sign it, then scan it, then email it seems long winded.. Given non-covid pts want notes and covid pts might want notes if off for more than 1 week.. OK - a colleague has just asked me what benefit is a video consultation over a telephone call. He’s not impressed by this rush to video. To be honest I’ve previously said the same - however on some of the test calls I’ve done I’ve been impressed with the video quality and personally I think it would allow me to assess a patient more reliably than just listening to them pant.“ “ “However.. I think there is more we could do and this could be the start of a revolution. “ “ “This article (https://lnkd.in/eTYWxMX) says they can measure heart rate from a video image. “ “ “I’ve come across articles saying you can measure pulse ox from images (https://lnkd.in/errRuJi) Im sure someone told me you can measure temp as well. “ “ “My iWatch will do an ECG and pulse rate and apparently pulse ox is coming too.” “ “Perhaps todays phone cameras need upgrading to do some of the above - and perhaps they need a way of uploading their data into the video chat room but how amazing would it be if I could video call someone - get their pulse, ECG, SaO2 and temp!

A while ago we moved most of our local practices on to Lexacom’s cloud solution (bulk deal/ simplified training and support). Been thinking about business continuity all weekend and in particular what if our admin staff and secretaries go down and we are really pleased as Lexacom allows us to share the typing across practices (any sec from any local practice can do anyones work if need be. “ “ “Alternatively we can setup a centralised local pool of secretaries who can do any practices work (an idea for our PCN anyway moving forward). We even have the option to electronically outsource the work to third party companies.” “ “From a prescriptions pov we are massively pushing people to nominate pharmacies (and think of choosing one who delivers or posts) for EPS so they don’t have to come into surgery or even go to a pharmacy. We are also keen to push repeat dispensing to minimise the amount of work we have to do, however when we’ve tried it before its never worked as well as it could… perhaps needs looking at centrally asap!” “ “#pcn #primarycare #gp #gp #nhs #practicemanagement

Been playing with the new accuRx video functionality for general practices. It’s really good. Major advantage is the patient doesn’t have to have an app the link they get sent opens in a browser. Thanks to Thomas Andrew Porteus I’ve worked out how to use my 2nd screen and webcam to do my side of the consultation on my desktop rather than my phone. Even better using the desktop rather than my smartphone allows me to share my screen so I can actually show patients their results and letters (if I need to). Also - the virtual room allows up to 4 in the conference so its possible that a ANP or similar doing a triage call could add in a more experienced GP for advice.“ “ “I also like their covid screening tool, the functionality could be useful post apocalypse to routinely ask patients much more about why they want an appointment in the hope you can deal with it without seeing them.” “ “Today it turns out they have introduced one time answers to messages - allowing patients to reply to messages sent to them.” “ “REALLY IMPRESSED! if you aren’t using it - you should check it out. ACCURX Video Consult

Ok - competition is good! Just been testing the new video consult module from accuRx - it works really well. Send a link from my desktop to my phone and the patients’ phone - click on the link and join a conference room with the 2 of us. only takes a minute to download the app and run it. on 4G both ends video quality and sound was good - downside is its on my phone - my data unless i can get on wifi - also have to hold phone or prop it up rather than use 2nd monitor but it didnt needs any pre installation of app the way iPlato does and I think they still have a 20min sync issue (need to check on this) #primarycare #gp #nhs #pcn #practicemanagement #digitalhealth

Thanks to Kieran Waterston and Victoria Barnes got iPlato Healthcare’ s video consult up and running yesterday. It’s quite simple and works well. Being honest (I usually am) the workflow and UI perhaps needs improving but we are getting it for our PCN partly to allow assessment of people at home. “ “ “They have some nice enterprise features coming which will help with Extended hours too allowing a central clinician to see patients from multiple practices. “ “ “- I have heard of AI that can read temperature, respiratory rate, heart rate and even O2 from a video feed if they could add this I may never need to see anyone ever again! “ “ “#coronavirus #pcn #primarycare #gp
