“(ie qualified doctors who are on a GP training scheme working towards being qualified GPs) shows that only 27% want to work full time in general practice long term. There are a variety of reasons for this and I’m not saying its a bad thing - I’m just aware that my surgery for the first time in its history had no applicants for a vacant post recently and one of our promising junior salaried GPs has just resigned to pursue a portfolio career. Is skill mix going to be enough? Do we value general practice? do we want continuity of care (how many get it now?) Do we need to understand why it’s so unattractive? Do we need to be radical and shorten medical school training? thoughts? #primarycare #generalpractice
My now bi-monthly DigitalHealth column is out: Some of my views on population health management (stratification and segmentation) and risk scoring in general practice. Its a huge topic and they only give me 800-1000 words so I’ve had to be brief but I think historically people have been getting it wrong by concentrating on just hospital admission avoidance and general practice has got it wrong by being too reactive (mainly to people with minor ailments) and not proactive enough. Recently there was a motion that General Practice should stop home visits - should general practice stop being the first point of call for trivial illness - is a hugely experienced workforce that could be taking on much more hospital work and delivering better health outcomes being wasted on seeing snuffly kids who will get better anyway? Should we be targeting those with the most health benefit to gain?“

My company, Howbeck Healthcare Limited, is developing a financial benchmarking tool and report for GP practices and are looking for more volunteer practices to test it out on, who will get it free. We sign confidentiality agreements with you and send a list of questions about your practice (that we are developing), you securely send us a copy of your accounts and we will produce a report and feedback on how you are doing compared to other practices. To include: where there may be opportunities for income generation, cost reduction and how your profits and costs compare to others. We have done this for over 15 practices so far and for some, it can be very interesting and informative. To be fair some don’t learn a lot as they are doing well already. We have found that even practices with specialist medical accountants benefit though practices who use non-specialists tend to benefit more. Get in touch if interested.

I’ve always liked the idea behind their product and its useful to catch up and be pumped by them for info on GP networks in the process :-).
They have a big private deal with BOOTS and they are starting to make inroads into the NHS with their “could it be a UTI” kit.
While useful I think the real benefit is getting patients to do their own ACRs perhaps at home. We know ACR is a big marker for complications in hypertension and diabetes. We know some patients don’t come in to have it done.
The ability to send a kit to someone and have them do it at their convenience appeals to me. (I think more CDM will move this way)
The in year cost savings is always the trouble as most of the benefits you get are in the long term versus the up front cost. Im suggesting we look at how much practice time/effort it might save and try and see if practices will fund it for that reason.

“Im hearing the phrase “GPs will be working at the of license” a lot. There is about to be an explosion of new staff in primary care. The new DES is part funding Physios, Paramedics, Physicians Associates and Pharmacists to work along side GPs doing some of their role. Great you might say, but are GPs up for leading & managing this team? GPs aren’t really taught this as part of their training? GPs sit in a room by themselves seeing patients, working in isolation. Ok there are consultants to ask for help and there are practice nurses who do chronic disease management and treatment room and in bigger practices GPs work in teams but it’s not quite the “consultant GP” role people are talking about.“

“What I’m not hearing is a lot of training for the GPs left working in practices who are going to have to lead and manage these teams. PCNs might want to think about spending some of their monies on training their staff to cope with this task? training companies might want to grab this opportunity?”

“The idea is simple. I want to be able to use my iPhone (or other) and take a series of pictures of a wound/lesion/rash/person. I want to be asked a few questions eg enter pt ID, perhaps which part of the body it is and I then want it to send those pictures via NHSmail (should be preinstalled on the phone ?can you check?) to a predefined NHS email address (or choice). It should then delete the images ie not store them on the device. Thats it..”

“We have been looking at referrals locally and there is a huge need to include simple photos with dermatology referrals but no easy way of taking them and sending them to the secretary or uploading to the clinical system that doesn’t involve buying kit or bringing the pt back. Just being able to email a pic to the secretarys email with the ID so they can match it to the referral letter is what we are after. NHSMail fails me again! Trying to share my NHSmail calendar with the university secretary that I work with (so she can see when im free for meetings) and it doesn’t work.. Email the Helpdesk and they reply sharing outside of Nhsmail isn’t allowed! - Why? I cant be the only person who works with a university or other organisation? I can now understand why my local hospital don’t use NHSmail and have gone direct to Office365 and all its added functionality. #generalpractice #primarycare #nhs #gp #DitchNHSMail

Good news from Mark Hashemi My local GP Alliance works with Technomed using their excellent 12 lead ECG service. All our practices use their kit. There is the option for patients to have ECGs done in their home or hub centre and the result get back automatically to the requesting GP. Across the federation all ECGs are now reported to the same standard and protocol, reducing the need for referrals to cardiology for interpretation. #primarycarenetworks #primarycare #ecg #cardiology #gp
Came across this fascinating article about a significant improvement in solar cell technology - then noticed I knew the the prof in charge - Prof Tony Peaker - when I was a medical student I worked as an IT support technician for his research department one summer vacation. Installing wordperfect 5.1 for ms-dos, Ventura publisher for GEM and Borland Paradox Database - brings back memories. He was a genius then - amazing to see he’s still at the cutting edge.
Good article. Its interesting that my local acute trust has refused to use nhs mail given some of its limitations and has implemented office 365 with access to its enhanced features. However I agree even full office 365 isn’t the perfect tool

Attended an interesting presentation from THE BENCHMARKING PARTNERSHIP LIMITED. They are branching out into PrimaryCare Pathology Demand Optimisation. Locally I’m working with the acute trust to reduce the number of inappropriate tests ordered by GPs (with some success) and we wanted to horizon scan. TBP have been doing this elsewhere and have also looked at and have a load of fascinating data on under ordering as well as over ordering. They have also done a lot of work on Diabetes care and can show some clear benefits in terms of HBA1C control. (I suspect the Hawthorne effect helps). One of the key benefits is they appear to be able to extract rich data and manipulate it from path lab systems and are getting good at presenting it as useful info.
Howbeck Healthcare has supported one of its GP Federations in Staffordshire to deliver the first of its kind in the country, the Staffordshire Physician Associate Internship to support the shortage of GPs. The uniqueness of the internship is it offers PAs to work across both primary and secondary care with a half day teaching session each week. 19 PAs have undertaken the internship, with the majority of PAs completing the internship offered full time positions in primary care.
The successes to date has been remarkable: RCGP endorsement; HSJ Values Award Finalist 2019; International paper submitted to WONCA (World Organisation for National Colleges and Academies for Global family doctors) for consideration and visits from national leaders and organisations from across the country

Im very impressed with the quality of the new ECG feature that the latest iOS has enabled. Very easy to do, very easy to create a PDF and email.
Some people are worried about the accuracy rates of the interpretation but this doesn’t bother me – 1. I think it will get better with more data and 2. I can use the PDF as a triage/screening tool.
Of course I think there is more they can do with it to help monitoring of dangerous drugs. See my previous post and my digitalhealth.net article
A team from Howbeck Healthcare Limited ran a 2nd network workshop yesterday to the forming GP network in the SMASH locality of South Cheshire CCG. They did the first network workshop in Northwich, Vale Royal CCG last week. The workshop goes over pre-extracted data from the practices on demographics/demand/activity and presents this to the network as the basis of a dialogue for collaborative working. They discuss what common pressure points they have, what interests and skills/expertise they may have, what schemes have been tried else where that they could do themselves, what pots of money there are to fund things and try to come up with a common vision and plan. The events went well. Some of the more cynical members were by the end of the event happily contributing to what to do next year as a network. The events were funded by NHS England as a pilot. Met with Gerald Blair from Clinisys this am - he’s the product owner for the ICE desktop order comms system we and a lot of surgeries use. Had a great end user feedback session on things that would make GPs lives easier and more productive and deliver better safer care to patients. #primarycare
