We work with practices and GP federations and realise that often there is a small startup or company out there with ideas/products that would solve their problems but perhaps because they aren’t as well known as the “famous” ones the connection and deal never happens.“ “ “We are also aware that some companies are keen to find clinical input, just to run their ideas past, to beta test their software on or to become early adopters. “ “ “We are planning to start listing innovative products and companies on our website (the page/section isnt live yet) and in our newsletters that go out to our 200+ GP practices and hopefully work with those that our practices find interesting. “ “ “So Get in touch if you’d like to be listed. Thanks for all the contacts re a mental health bid - the deadline is short and we are looking at whether any will be workable up in such a short time. There will be other bids..so great to make new contacts - speaking of which - is anyone working in the AI field? We are convinced there are a range of applications for AI to make GPs lives easier and would love to develop a relationship with someone working in this field even if they aren’t in health yet. We work with over 200 practices and have programmers who understand the primary care systems APIs what we need is some experts in AI.

really like this kickstarter project - off grid mesh networks - wouldn’t a few of these be good on school trips to the wild - or even in rural areas. seems a shame it can only cope with 2 bounces and unless using them in a group you’d want a decent chance of someone else having one for it to work??? Should the NHS invest in google adwords? signposting away from GPs is a hot topic at moment. we have practice websites trying to do it - nhs choices - self care websites but in my experience most people just google - they don’t go to a website. Try googling UTI or thrush or ear ache or tonsillitis or chickenpox - why isn’t there a nhs sponsored adwords box popping up - adwords campaigns can be localised to postcodes and areas, you could have local information - which chemists are open - what minor ailment services exist - even just links to self care. ok sometimes nhs websites come near the top of the list but a big adwords box with NHS all over it?

Video met this company today and had a demo of their product. It looks really interesting and i’m connecting them with our local Social prescribing hub provider Pathways Community Interest Company (who are excellent btw) to see if there is any synergy/scope for some joint working.
Lets save a fortune for the NHS by getting rid of post! If every person was given for free a secure private online mailbox that could receive authorized “post” in such a way that was secure and confidential - linked perhaps to apps or SMS so that you got a notification or SMS that a message was waiting for you - perhaps with read receipts ability to reply and a built in calendar. Wouldn’t this save millions? My surgery spent £10K on posting last year - there are 8000 GP surgeries in England. There are 200+ hospitals who write out to patients with appointments and scan results. Millions is being spent on post needlessly. often it goes to wrong address or doesn’t arrive in time - or the person is on holiday. If i was sat on a yacht in the med and my phone pinged and i logged into my mailbox and saw my scan appointment letter how much better would that be than arriving home and finding id missed it? you could fund this by a nominal charge - would my surgery pay £1K to save £9K you bet (at 8000 surgeries that makes this an £8Million business for primary care alone!) . Would opticians, dentists, physios - others Taxman etc pay to use the system? probably if it saved them money - especially if it was authorized not spam senders only.
Can the NHS learn from Amazon? In the book the “Amazon Way” one of the key messages is that you can deliver an amazing customer experience by automating a process and it also takes real cost out of the system. Instead of having an expensive call center Amazon use their website to handle returns and have designed & refined it to cope with most eventualities. In GPland annual reviews are a huge burden both for practices & patients. BP, Asthma, Diabetes, COPD, etc. Most of these require a practice to operate some form of call/recall system, the patient to take time off work, to attend for a bloods then come back for the appointment. In most cases this might be normal and they are told to come back in a year. People have talked about using Skype but that still involves an appointment and a person. A real disruptive change could be achieved if we could automate these reviews on a website. Blood testing kits now exist that can be posted to the patients house so they can do their own bloods on capillary samples, home bp machines, ecgs and other kit are now cheap and could be sent to a person. Using the Pareto principle if the NHS could move 80% of this work out it would free up huge amount of resource and the lesson from Amazon is the customer loves the flexibility and convenience.
Here’s an idea for a website/service that could revolutionize patient care - its needs a catchy name. “What side effects are my drugs combined giving me dot com”. Its relatively easy to find out the side effects of any one drug. each drug has side effects ranked in order. frequent, rare, very rare etc. I come across patients on 20, 30 or more meds. I’m a great believer in stopping meds and only using the least you can get away with. I think a lot of symptoms people suffer with are side effects that stopping meds would help and that’s the answer not giving more to counter act them. However I cant find anywhere a website or tool where entering all the meds a person is on, will combine all the side effects possible and rank them in order of likelihood. The info exists it just needs a programmer to build. We spend our lives changing drugs from one to another because its slightly cheaper. Stopping drugs that aren’t doing any good and may be causing harm is the way to save money!
We need more anger emojis! The inuits allegedly have over 80 words for snow. I have at least 80 types of anger - i cant cope with 1 emoji! The anger when the water goes off for the 3rd time in 8 weeks, the anger when some idiot leaves their fog lights on and its not foggy, the anger when my nhs smart card fails 5 mins before surgery, the anger when a solicitor tries to get a report done for cheap, the anger when the Vat advice company give rubbish advice, the anger when you are driving along at 60 and some idiot pulls out in front of you and does 40 slowing you down, the anger… i could go on and thats just this week! i NEED more ANGER emojis!!! #INEEDMOREANGEREMOJIS

Despite the IT issues.. really pleased that our GP Federation DVT service has successfully finished its first week! Any patient from 30 surrounding practices with a suspected DVT are referred to my surgery - we do the Ultrasound and if positive their bloods using a Point of Care machine from Siemens then dose them with a NOAC. Its all done in the community by GPs and my GP federation is managing the £250K contract - generating a saving for the CCG. Satisfaction is high and despite the IT everything has gone well. Met with Richard Cardow from Qardio discussing their MD portal for their products Qardioarm, QardioBase and Qardiocore. We were discussing how to get primary care interested in purchasing the devices and different potential models of use. Hes left me a couple of samples to play with and I’m trying to create a possible case study possibly in heart failure patients.
£100M for GPs to work in A&E in the budget. with ?200 trusts thats £0.5M per hospital - ok - im considering offering myself - 365 days non stop being a Wall (if you know that reference have 5 points) cant be worse than being a house officer in the early 90s!
We have a vacancy coming up for a IT person to help us support the GP federations and practices we work with. We are working on the job description but its a bit of everything - we need someone who can liaise with the CCG and CSU supported by us - someone who can run projects - deal with all the IT companies we work with - provide IG expertise, liaise with practices, do some training both 1:1 and 1:group, get involved in sorting problems at a practice and federation level. We have a great team already. Knowledge of EMIS useful. Experience of primary care useful. We are based in south cheshire and cover across the A500 to Stoke looking after >100 practices. email me!

My GP federation is planning a (not for profit) conference on Tuesday 13th June, in Crewe Cheshire aimed at GPs and GP Federations; aiming to share innovative ideas & success stories, especially IT innovations. (If successful we aim to replicate around the country). It will be free to attend for delegates - and we will advertise around the NW & midlands (crewe is only 1h35 from London so who knows!)
